Please include the following information on your purchase order or correspondence:

• Purchase order number
• Shipping address
• Billing address
• Catalog number and description
• Name and phone number of purchasing agent
• Name and phone number of end user
• Quotation number (if applicable)
• Vendor name (BioTime, Inc.)
• Vendor address:
